Veterinary Voice Dictation: From Speech to a Reviewable SOAP Note

The value of voice dictation in a veterinary examination goes beyond turning speech into text. The note must belong to the correct patient, preserve what was said and fit a reviewable record structure. The endpoint is not generated text; it is a record the veterinarian has checked.

This guide explains the path from a spoken note to a SOAP draft and the checks before saving. It is not a version announcement or an accuracy promise covering every recording environment.

Dictation and a SOAP draft are different tasks#

Dictation transcribes speech. A SOAP draft organizes information into history, findings, assessment and plan. A correctly transcribed sentence is not necessarily attached to the right patient or placed in the right section.

Make the source of the information clear#

Rather than listing findings without context, state whether information comes from the owner, today's measurement or a previous record. Say when something has not yet been assessed.

Fictional documentation example:

Owner reports reduced appetite for two days. Today's weight is 4.2 kilograms. The date of the earlier weight needs checking. Cause not yet established. Clinician assessment and follow-up plan still to be completed.

This example does not recommend treatment. It shows how to distinguish information, uncertainty and pending work. The same approach is useful for typed notes.

Check the draft in six places#

Do not fill a gap with an assumption merely to make the draft look finished. Return the question to the clinician or complete the information appropriately. The RCVS advice on AI also emphasizes human review of generated records.

Try the actual working environment#

Use the device and microphone intended for the clinic with a few example notes that contain no personal information. Include short sentences, measurements and negative statements. Record what needs correcting.

This is an exercise for learning the interface and understanding your working conditions, not a clinical accuracy study. Do not assume that all background noise is removed or that every dictation function works without connectivity. Confirm current requirements during a product discussion.

Your clinic's process should make clear who can start a recording, where it is appropriate and how information will be handled. Focus on information needed for the purpose rather than unnecessarily recording broader conversations.
